Tennessee prison commissioner Frank Strada resigned after a botched execution of death-row inmate Christa Pike left her in critical condition. The failed attempt marks the second execution failure in Tennessee this year, prompting Governor Bill Lee to halt further executions for the remainder of the year.
Strada stepped down days after the failed lethal injection on Wednesday, which left Pike unconscious, intubated, and on a ventilator. Pike, 50, was convicted in 1995 for the murder of Colleen Slemmer. Her lawyers accused officials of ignoring medical warnings about her small veins and blood disorder.
Governor Bill Lee has ordered an independent review of the execution attempt at Riverbend Maximum Security Institution. Lee said Strada's resignation is in the best interest of Tennesseans as the review proceeds. The state has halted executions for the rest of the year following the second failure in 2026.
